Medical Home Care Coordination
At Pediatric Care Associates, we're dedicated to providing comprehensive care for your child, focusing on their well-being beyond just medical diagnoses. As your patient-centered medical home, we prioritize addressing your concerns and needs with compassion and expertise.

Our providers adhere to evidence-based care guided by the American Academy of Pediatrics (AAP) guidelines, ensuring your child receives the best possible treatment. We integrate the latest scientific research, clinical experience, and your preferences into our practice to make informed decisions about your child's care.

Beyond medical concerns, we're here to support your family's mental and emotional well-being, address financial insecurities without judgment, and provide education and supportive services as needed. Our goal is to help you and your child achieve the highest level of health and wellness in all aspects of life.

As part of the Pediatric Physicians Organization at Children's (PPOC), we have a strong collaborative relationship with Boston Children's Hospital. This partnership enhances our ability to provide quality care, supported by additional resources and expertise.

We offer telehealth visits for your convenience, allowing you to seek treatment from home when appropriate. Our care coordination services ensure that your child's needs, whether medical, social, developmental, or financial, are addressed comprehensively.

Through this program, we advocate for all patients, especially those with complex medical, developmental or behavioral health challenges. We provide one-on-one education and outreach to help our patients and their families connect with services and resources beyond our practice such as our medical home asthma program, farm to family, providing autism supports, guardianship assistance and much more.

Thank you for entrusting us with your child's care. We're committed to providing the support and resources your family needs to thrive.
